Considering that what someone considers 'fun' varies so wildly, missions in City of Heroes could be run in one of 4 (or 5?) increasingly difficult settings, with more difficulty having more rewards, but tougher opponents... Risk vs. Reward.
This was scratched for Champions Online since it has a more WoW-like approach (open world, outdoors kill 10/click 10 type of missions), and every time anything changes, you'll have a group of people flaming the forums. If something is made harder/nerfed, forums are ablaze in that 'I don't feel like a superhero anymore'. If something is made easier 'This game is too easy, pushing one button to win is not fun!'.

I would like to set my difficulty high, as I enjoy challenging fights (with bigger rewards), but I understand other people prefer easy fights. Will we be able to control difficulty settings for missions like in City of Heroes?

My understanding of how difficulty in STO will work is that Each mob will Scale to Your level. I'm not sure how this will work with fleets and such but i assume it would take the "average" level of the fleet.

The more skill points you have as a player, the harder the mobs you fight will be.

I believe this was covered briefly in this weeks Dev Chat, i'll go find a link to the transcript.

But then, it's the same for everyone with the same level. Basically, if we both have a level of 'X', we'll encounter the same opposition. What I'm saying is, my definition of a fun fight might not be the same as yours. In CoH, I could select among 4 or 5 settings and everybody was happy, if you like easier fights, select 'easy' setting and if you prefer harder fights, select 'hard', with harder settings yielding better rewards, but in CO there's only one setting (and seems like nobody is happy).

I don't think we'll see harder difficulty shards, but I would really like to see difficulty settings that determine the strength of the ship/enemies that spawn on missions similar to what City of Heroes did.

The main difference between CO and CoH is the fact that CoH is 95% instance while CO is mostly open world in regards to missions. It is a lot harder to allow the players to scale their missions when the missions are based around the open world and not in a private instance.
