So what would you like to see happen to you if your ship is destroyed. Do you have the ability to escape in some type of escape pod? Would you have to ability to be rescued or beamed aboard another players vessel? Or even captured by being beamed aboard an enemys vessel?
If you're killed in battle, where would you reappear? Some starbase or space station? Would you like to see any penalities for being killed like demotion in rank or being prohibited from commanding the type of vessel you were destroyed in?

Topic has already been done.
In any case, I believe that Cryptic has stated that they want the game to be fun, and as such the penalty for losing your ship or your death as the captain will be minimal if at all existent.
I personally think that if you were to lose your ship or die that as minimal penalty should be something like losing a tech you acquired or something like that maybe. Yes, it would be bothersome if you keep dying a lot, but it would also make players think more before just going out and getting into random fights with no meaning with other players.
And most likely you would respawn at either the nearest starbase or the last base you visited with your last vessel in use.

for PvP to exist in a game, which it is expected to exist in STO, the death penalty has to be minimal if not non-existent. i also imagine that when people die, its going to be in a fairly non-spectacular fashion, or else, everyone's computer would crash if 5-10 people died at the same time in pvp. you'll have to respawn somewhere that is away from the action, and you might have to pay alittle currency to repair your ship.
